In order to expedite the check in process, please review and complete all the documents in the New Patient Packet and the HIPPA Acknowledgment prior to your appointment. Please bring your completed forms, insurance cards, driver’s license, and copies of any recent laboratory tests or imaging reports.

You may, if easier for you, have them faxed to our office (Fax: 805-370-1984) before your scheduled appointment.
